Medical Release of Information
The Health Information Department's hours of operation for Medical Release of Information are Monday - Friday from 7:30 am - 5:30 pm The process for requesting a patient's medical records includes obtaining a medical release of information authorization that is signed and dated by the patient.
A patient can also request the release of medical information by faxing a signed/dated authorization or by sending the HIM department a letter in writing. The authorization must include the medical information being requested, for example, recent lab studies, operative report, etc. In addition, the request should specify the purpose of the disclosure, to whom the information is being released as well as address, telephone number, work number, date of birth and a copy of identification.
The normal response time for completing any medical request is 5 business days from receipt of the request. There is no charge for portions of records, however, there will be a fee if your complete medical record must be copied. For specific questions regarding release of custody cases, protected medical information, psychiatric records and those cases that require further clarification, please call the Medical Records Specialist at 919-938-7705 or the Director of HIM at 919-938-7516.
Birth Registration
The Health Information Department's hours of operation for Birth Registration are 8:30 am - 5:00 pm, 7 days a week. All births are registered in compliance with State Vital Records guidelines of 5 days following the birth of the baby. The baby's information is obtained by interviewing either of the parents for the name of the infant and his/her date of birth. In addition, the parent's demographic information is obtained to ensure accuracy of the birth record. In the event the parents are not married, a paternity affadavit must be completed and signed by the father for his name to be printed on the birth certificate.
Once the birth record is complete, the mother signs the original record that is sent to the State Vital Records. The parents must then send the state a money order to obtain the original birth record. The parents can also obtain an original birth record from the county Register of Deeds office located at the court house.
